If you feel you have a problem with gambling and would like to request that Crisis excludes you from receiving gambling related marketing material, please complete and submit this online form.

Please gamble responsibly. If you or a family member feels that they are experiencing problems with gambling, you can seek advice and support from trained counsellors by visiting www.begambleaware.org or call The National Gambling Helpline on 0808 8020 133 (lines open 24 hours a day, 7 days a week).

We will use your email to send you an acknowledgment of your exclusion from all Crisis raffles.
